Please ignore my request for help, I have recompiled my kernel, deleted the existing /etc/printcap and re-run apsfilter setup, and somehow managed to get everything working. Now all I have to do is work out what went wrong the first time!

Sounds like the printer daemon (maybe lpd, LPRng, gnulpr or CUPS) was dying for some reason.

Or a lock file in /var/spool/lpd?
