I ended up with an M-AUDIO MIDISPORT 2x2[1]. My setup:
Keyboard <--midi din--> Midisport <--midi din--> ExternalSynth <--usb--> laptop
I only briefly played with Rosegarden under Linux, as I remember there was some firmware required[2].
Most of the time I don't bother with the laptop, so it's passes Midi-In to Midi-Out (this can be toggled on the box itself).
I did have a play with taking Midi-In in via USB to Rosegarden, then back out over USB and out again to Midi-Out and the external synth.
