However, I found newer forum posts by people confirming that they were able to change the IPS tag without having to pay. So it's entirely possible that 123-reg scrapped the hidden fees in the mean time.
Odd that I wasn't aware, but then it's 4 years ago so maybe I was at the time. Just one of many reasons to be moving away, though.
