I think I used GIMP to edit icons, maybe using netpbm to convert them to winicon or whatever was needed.

Theres also an online thing called El Jay Icon Maker which I use a lot - there is a firefox plugin so you can rightclick on any image and it prefills the form, then gives you crop, resize, add border and text etc

if you're an LJ user you can have it upload directly to your live journal page which is quite funky (but if you're not, just save the image).
... but that just converts images to icons doesn't it? I can do that already quite easily with the netpbm tools. What I want is a Paint type program specifically for icons. Apart from anything else it would be useful to 'touch up' icons I've converted from images.
