There is a questionnaire about software use and development online at http://www.postgrad.isipartnership.net/~aschofield/FOSSsurvey.htm
It's being carried out by Andrew Schofield, webmaster of the Linux Society and User Group based at the University of Salford, as part of his PhD studies. It's one page, doesn't seem to need scripts, cookies or CSS and if you provide your email address, you'll be sent a copy of the results when it's completed.
